Community Health Partnership of Illinois (CHP) is committed to improving the health and well-being of migrant and seasonal farmworkers. We support these communities by providing un hogar de salud, a "wellness home" where workers and their families receive quality medical and dental care from a team of dedicated, bilingual-bicultural professionals, in an atmosphere that fosters a sense of belonging.

For over 30 years, CHP has provided a broad range of primary health care services through a unique delivery model based on collaborations with the public and private health sectors, social service agencies, academic institutions, advocacy organizations, governmental and private funders, and, above all, the farmworkers we serve.
