CHP has five health clinics that serve migrant and seasonal farmworkers and their families. We operate a nurse-managed health program for farmworkers that stresses outreach, health promotion, and case management.

CHP offers all medical and dental services according to a sliding fee scale. However, the vast majority of patients qualify to pay only a modest, voluntary co-pay for doctor visits, diagnostic services and prescription medicines.

"Community Health Partnership's strong commitment to addressing health issues with farmworkers themselves, and their creativity and compassion in providing services, combine to make them exemplary within the migrant field."

Primary Health Care

In response to the critical need for basic health care, CHP has established five health clinics in separate service areas that provide comprehensive health care tailored to meet the special needs of this unique population. In addition to our core services of medical care, prescription drugs and diagnostic services, CHP health teams also provide outreach, health education and screening, and case management services. We conduct evening health clinics for migrant workers who are unable to take time from work for medical appointments.

Oral Health Care

In 2001 CHP opened a three-chair dental clinic in Aurora that offers comprehensive dental care to over 3,000 farmworkers and family members in our service area. CHP also operates a school-based dental program that brings comprehensive dentistry to 1500 migrant children enrolled in Migrant Education and Migrant Head Start Programs throughout Illinois. The hallmark of our oral health program is a unique partnership of those who work with us, including a variety of private dentists and hygienists, and the University of Illinois College of Dentistry.

Cancer Prevention and Reproductive Health

The incidence of breast and cervical cancer in the migrant population has been on the rise in recent years. CHP offers migrant women a full range of reproductive wellness education, counseling and clinical services, including annual physicals, Pap smears and mammograms.

Agricultural Health and Safety

CHP is experienced in health and safety research, as well as intervention programs for agricultural workers and their employers. We have been involved in various projects funded through the National Institute for Occupational Safety and Health (NIOSH) the Great Lakes Center for Occupational and Environmental Health, and the United States Environmental Protection Agency. These projects address pesticide safety, eye and back injury prevention, and childhood environmental health issues.

HIV Prevention

In response to the rising number of HIV cases within migrant populations around the country, CHP developed a comprehensive program of HIV prevention education, risk reduction counseling and HIV antibody testing. Our promotores de salud play a pivotal role in reaching migrant workers in remote rural areas with the tools and information necessary to reduce HIV and other sexually transmitted infections.

  Community Outreach and Health Promotion

Bringing the programs and services we offer to those who most need them is fundamental to our mission. Our bilingual, bicultural community health workers and promotores de salud provide outreach at migrant labor camps and farms. Outreach activities include family registration, health screening and education, and assistance with other services available to migrant farmworker families, including KidCare, Medicaid, Food Stamps, WIC and Migrant Education and Head Start programs.
