Our CHP Mendota Medical-Dental Clinic staff has been spreading holiday cheer again this year, in partnership with the Mendota Area Chamber of Commerce Ambassadors and Youth Service Bureau of Illinois (YSB). As part of "Operation Elf Action," we collected toys and clothing to donate to Mendota-area families, offered free wellness checks for glucose and blood pressure, and registered new patients at the clinic.
We are excited to announce that Elizabeth Rios, nurse practitioner at our Harvard Area Community Health Center, has been honored by the Migrant Clinicians Network as one of the "30 Clinicians Making a Difference." Elizabeth first started working with CHP as a student, where she had a clinical rotation at our Woodstock, Illinois clinic. Since then, she has moved on to our Harvard Clinic, where she has been caring for patients with her blend of warmth, knowledge, and caring.

And when they gain your trust, it means they'll come back...and they'll let other people know how safe it was." - Elizabeth Rios We are pleased to announce that Victoria Chavera, the Immigrant Family Resource Program Specialist and Patient Services Specialist at our Harvard Area Community Health Center, has been honored with the Farmworker & Landscaper Advocacy Project's (FLAP) 2014 Beckman Advocate of the Year Award.
Victoria has worked with Illinois' agricultural workers for over 25 years, and this award recognizes her outstanding advocacy work in the field. We are thrilled to have her as part of the CHP family, and are grateful for all the work she puts in, day in and day out, serving our community. Our Harvard Area Community Health Center will be getting a major upgrade, thanks to a $1.2 million dollar grant we received from Governor Pat Quinn's Illinois Jobs Now! capital construction program.
This grant will allow us to purchase an existing building, renovate two suites, and add a dental clinic, additional exam room, and designated space for behavioral health services. We are humbled and grateful to receive this generous capital grant, which will benefit countless individuals and families in the greater Harvard area for many years to come. Our Mendota Medical-Dental Clinic has earned National Committee for Quality Assurance (NCQA) recognition as a Level 2 Patient-Centered Medical Home. So what is a Patient-Centered Medical Home? According to the NCQA:
"A patient-centered medical home is a model of care that strengthens the physician-patient relationship by replacing episodic care with coordinated care and a long-term healing relationship. Each patient has an ongoing relationship with a personal physician who leads a team at a single location that takes collective responsibility for patient care, providing for the patient's health care needs and arranging for appropriate care with other qualified clinicians. The medical home is intended to result in more personalized, coordinated, effective and efficient care." We are proud of this recognition and what it means for the staff and patients at our Mendota Clinic. Please join us in congratulating them on this distinction!

We are delighted to announce that Gloria Rivera, LPN, has been honored with the Illinois Primary Health Care Association's (IPHCA) Ramona Lopez Award at the IPHCA's 31st Annual Leadership Conference held October 2-4, 2013. The Ramona Lopez Award recognizes individuals who have dedicated significant time and energy to their organizations and who have continuously worked to improve the quality of care, and access to care, provided to patients.
We are grateful for Gloria's dedication to CHP over the years, and hope you'll join us in congratulating her on this distinction. Gloria has been with the CHP family since 1997, when she started as a community health worker/CNA. Today, she is the Director of the Aurora Medical Dental Clinic.
